The Role

Circadia Health is expanding its clinical delivery infrastructure and establishing physician-owned professional corporations (PCs) across multiple states, supported by a management services organisation (MSO).

The company is seeking experienced physicians to serve as the owner and President of one or more affiliated medical groups. This is a governance and clinical leadership position rather than a full-time clinical role.

The President will provide clinical oversight, regulatory compliance and professional supervision for an NP-led care model serving skilled nursing and home health patients.

Key Responsibilities

Clinical Governance

• Serve as sole or majority shareholder, director and President of one or more state-specific professional corporations.
• Approve clinical protocols, quality standards and scope-of-practice policies for NP-delivered care.
• Oversee care models including remote patient monitoring, chronic care coordination and transitional care.
• Execute and maintain collaborative practice or supervision agreements with nurse practitioners as required by state law.
• Serve as, or designate, Medical Director for applicable programmes.
• Sign off on clinical policies, standing orders and care pathways.

Clinical Quality & Compliance

• Oversee credentialing, peer review and clinical quality committees.
• Maintain Medicare enrollment for the professional corporation and reassignment of billing rights consistent with programme requirements.
• Participate in governance meetings, audits and payer or regulatory inquiries as needed.
• Retain exclusive authority over clinical decisions while partnering with the MSO on compliant operations.

Organisational Oversight

• Partner with the MSO on operational matters while maintaining compliance with corporate practice of medicine (CPOM) requirements.
• Provide professional supervision for the NP-led care model across applicable entities and states.

Qualifications

• MD or DO with an active, unrestricted medical licence.
• Willingness to obtain licensure in additional target states; licensure support and costs are covered.
• Active, unrestricted DEA registration.
• Board certification in Internal Medicine, Family Medicine, Geriatrics or Physical Medicine & Rehabilitation preferred.
• Current Medicare enrollment or eligibility, with no revocations, opt-outs or payment suspensions.
• Clean compliance profile with no OIG/SAM exclusions, state board discipline or material malpractice history.
• Experience supervising NPs and/or serving as Medical Director in post-acute, SNF, home health, telehealth or value-based care settings.
• Comfortable with physician-owned PC arrangements, including stock transfer restrictions, succession agreements and MSO management structures.

Preferred Qualifications

• Prior experience as a PC owner for a telehealth, RPM or digital health company.
• Multi-state licensure, particularly in California, Texas, New York, New Jersey, Ohio and other CPOM-strict states, or participation in the Interstate Medical Licensure Compact (IMLC).
• SNFist or post-acute Medical Director experience.
• Familiarity with PDPM, RPM/CCM billing codes including 99453, 99454, 99457 and 99458, and CMS Conditions of Participation.
• Experience with quality programmes, utilisation management, peer review and payer credentialing.

Time Commitment

10+ hours per month, scaling based on the number of states and entities supported.

Compensation & Benefits

• $4,000/month fair-market-value retainer to start.
• Retainer may scale based on scope, number of entities/states and time commitment.
• Additional compensation for clinical oversight time beyond the baseline scope.

Compensation

$4,000/month starting retainer, with additional compensation for clinical oversight beyond the baseline scope.
