Sagara Health is building AI specifically for care management through its NexCM platform. The platform captures care-management conversations, transcribes them, identifies relevant information and converts that information into structured documentation and follow-up actions.
The goal is to reduce the administrative burden on care managers so they can spend more time supporting patients and members.
Sagara was founded by an experienced managed-care leadership team with more than 70 years of combined experience across Medicaid, Medicare Advantage, health-plan operations, value-based care, healthcare technology and care management.
The team has previously built and scaled health plans, provider networks, healthcare operations and technology programmes across major U.S. healthcare organisations. Sagara is now moving into an important commercial stage, with initial pilot activity beginning and plans to expand across health plans, ACOs, at-risk providers and other care-management organisations.
Sagara is seeking to build relationships with experienced healthcare leaders who understand the operational challenges facing care-management organisations and can contribute strategic perspective and long-term alignment.
The engagement is strategic rather than operational, with opportunities to contribute expertise across healthcare strategy, commercial growth, payer and provider relationships, healthcare technology and AI, strategic partnerships and company development.
Care managers play an important role in improving outcomes, coordinating care, closing gaps and supporting members, but significant time can still be spent documenting conversations, entering information into systems and completing administrative follow-up.
NexCM is designed specifically around this workflow:
Care-management conversation → transcription → structured information extraction → care-manager review → structured documentation → follow-up actions → existing care-management systems
Rather than applying general-purpose AI to healthcare, Sagara is focused on a specific workflow its founding team understands deeply.
Sagara is moving from product development into real-world commercial validation, with initial pilot deployment approaching and a broader enterprise adoption strategy being developed.
